4. Payment terms: - Payment for all 1:1 sessions is due at end of session. If
an insurance company is being used, then any excess which is not being
paid by the insurance company, is due within 7 days of notification.
Payment for all seminars and workshops is due no later than 28 days
following date of invoice.
5. Equilibria Health (Glasgow) Ltd. reserves the right to charge interest at
4% above base rate from the due date until paid if payment is late.
6. Citations to court:
Due to the preparation required ahead of a court appearance, should the attendance of our psychologist no longer be required, the following charges will apply:
Cancel more than 21 days before court appearance
0% of total invoice amount payable
Cancel between 15-21 days before court appearance
25% of total invoice amount payable
Cancel between 8-14 days before court appearance
60% of total invoice amount payable
Cancel between 0-7 days before court appearance
100% of total invoice amount payable
Travel and accommodation charges will be invoiced separately
7. All patients must fill out the Consent & Disclosure form that can be found on the new-patient page. Your treatment or assessment can be refused if you do not complete this form.
8. Equilibria Business contracts all feature a 3 month trial period. After this trial is over the contract is for 12 months. We will contact you toward the end of your year with us about renewing your plan.
Terms & Conditions
2. A cancellation charge will apply for any 1:1 session, seminar or workshop
that is cancelled unless cancelled by Equilibria Health (Glasgow) Ltd..
This will be as follows:-
1:1 Sessions:
Cancellation less than 36 hours (or non-attendance) will be charged at
the full cost.
Workshops and Seminars:
Cancellation more than 28 days before the scheduled date:
No Charge
Cancellation between 14 and 28 days before the scheduled date:
Charged at 50% of cost
Cancellation 14 days or less before the scheduled date:
Charged at 100% of cost
1. Equilibria Health (Glasgow) Ltd. will provide 1:1 sessions, workshops or
seminars at the place and time arranged.
