ENS Service User Agreement

3. Eligibility

 

To use our services, you must:

 

• Be at least 18 years of age.

• Provide accurate and complete information;

• Have legal capacity to consent to treatment;

• Comply with this Agreement.

4. Nature of Services

 

4.1 Healthcare Services

 

Equilibria Health provides professional clinical opinions and treatment recommendations based on information available at the time of assessment.

Healthcare outcomes cannot be guaranteed.

A diagnosis, treatment recommendation, prescription, or shared care recommendation may not be appropriate in every case.

 

4.2 Assessment outcomes and Clinical Decision Making 

 

Clinical assessments are undertaken to determine whether diagnostic criteria are met and to identify the most appropriate treatment recommendations. 

Booking or attending an assessment does not guarantee:

•    An ADHD diagnosis;

•    A prescription;

•    Acceptance into medication titration;

•    Acceptance into therapeutic treatment

•    Acceptance by your GP into a shared care arrangement;

•    Eligibility for workplace, educational, or disability accommodations.

 

Clinical decisions remain solely the responsibility of the treating clinician.

 

4.3 Emergencies

 

Equilibria Health is not an emergency or crisis response service.

 

If you believe you are experiencing a medical emergency, psychiatric crisis, suicidal thoughts, or immediate risk of harm, you should contact:

 

•    NHS 999

•    NHS 111

•    Your local emergency department

•    Emergency services

 

immediately.

5. Service User Responsibilities

 

To support the safe and effective delivery of care, service users are expected to:

5.1 Provide Accurate Information

 

You must provide complete, truthful, and accurate information regarding:

•    Medical history

•    Mental health history

•    Current medications

•    Substance use

•    Cardiovascular history

•    Previous diagnoses

•    Relevant risk factors

You agree to cooperate with reasonable requests for collateral information and the release of relevant healthcare records where these are considered necessary to support safe and accurate assessment.

5.2 Update Information

 

You must promptly notify us of any significant changes to your health, medication, contact details, or circumstances.

 

5.3 Attend Appointments

 

You are responsible for:

 

•    Attending scheduled appointments;

•    Ensuring a private and appropriate environment for remote consultations;

•    Arriving on time.

 

Repeated missed appointments may result in discharge from services and charges. 

6. Assessments and Clinical Decision-Making

 

All assessments are conducted according to current clinical standards and professional guidelines.

 

Clinicians may:

 

• Request additional information;

• Require collateral information from family members or third parties;

• Request previous medical records;

• Recommend alternative diagnoses;

• Decline diagnosis where diagnostic criteria are not met.

Clinical judgement remains final.

7. Prescribing and Medication Services

 

7.1 Clinical Discretion

 

Decisions regarding medication are based on a comprehensive clinical assessment and consideration of the service user's needs, medical history, risk factors and treatment goals.

Medication will only be prescribed where it is considered clinically appropriate and safe to do so.

Attendance at an assessment or participation in treatment does not guarantee that medication will be recommended or prescribed. 

 

7.2 Controlled Drugs

 

Many ADHD medications are controlled drugs regulated under UK law and require additional safeguards to support safe prescribing and service user safety.

 

 If prescribed medication, service users are expected to:

• Use medication only as prescribed and in accordance with clinical advice;

• Store medication securely and out of reach of children or other individuals;

• Not share, sell, transfer, lend or otherwise misuse medication;

• Inform the service of any concerns regarding side effects, loss of medication, misuse, or difficulties adhering to treatment;

• Attend required monitoring and review appointments;

• Participate in physical health monitoring where required;

• Comply with the requirements outlined in the Medication Management Policy and Controlled Drugs Policy.
   

Misuse, diversion, repeated loss of medication, failure to engage with monitoring requirements, or other significant safety concerns may result in prescribing being reviewed, suspended, or discontinued.

7.3 Monitoring Requirements

 

To support safe prescribing and ongoing treatment, we may require a some or all  of the following information, :depending on the medication prescribed and your individual clinical circumstances:

 

• Blood pressure readings

• Heart rate measurements

• Weight monitoring

• ECG results

• Additional investigations where clinically indicated

 

Monitoring is ordinarily undertaken in person at  our Glasgow clinic. 

Where attendance at the clinic is not reasonably possible, alternative arrangements may be considered. This may include obtaining monitoring information through your GP, another healthcare provider, or an external service commissioned by Equilibria Health. Such arrangements will be considered on a case-by-case basis.

Service users are expected to engage with all monitoring requirements relevant to their treatment. Failure to participate in required monitoring may result in prescribing being reviewed, suspended, or discontinued where it is not considered safe to continue treatment.

 

 

7.4 Shared Care

 

Shared care arrangements remain entirely at the discretion of:

 

• Your GP;

• Your NHS practice;

• Relevant healthcare providers.

 

Equilibria Health cannot guarantee acceptance of shared care requests.

Shared care arrangements may be withdrawn by a GP or NHS practice at any time and are not under the control of Equilibria Health.

 

7.5 Annual Review Requirement 

All service users prescribed ADHD medication are required to attend an annual specialist review.

This requirement applies whether medication is prescribed privately or under a shared care arrangement with a General Practitioner.

Failure to attend required reviews may result in prescribing being suspended or discontinued until a review has been completed.
